Overview
Faculty, Administrative Professionals and other Non-Classified employees located in Guam are eligible to enroll in any Anthem medical plans, and GeoBlue is contracted to administer Anthem’s healthcare services in United States territories, including Guam. While the claims processing and how to access care may be different, the same services are available to all employees. Employees residing in Guam can enroll in the Green, Ram-HDHP or Aggie Orange plans. The Colorado Pathways plan does not offer an available network.
After enrollment, each employee receives a personalized user guide for their health care providers explaining how to submit claims for guaranteed payment, obtaining authorizations, if needed, and GeoBlue contact information. Preferred and network providers can be located on the GeoBlue/Global Core website.
It is important to understand that for most medical services, no benefits are payable until the deductible is satisfied. (This does not apply to covered preventive services, which are not subject to the annual deductible and are covered in full for any provider in Guam.) Contact Human Resources for more information.
How does the medical plan work?
After enrollment, you will receive a personalized healthcare provider guide from GeoBlue indicating how to submit claims for guaranteed payment and obtaining authorizations, if needed.
After scheduling an appointment, but prior to the appointment date, you should contact GeoBlue at 800-810-2583. They will contact your network provider to give them the “pay agreement” letter and details regarding confirmed coverages for you.
If you see a provider without contacting GeoBlue first or if you do not bring copies of your provider guide/letter, you may be required to pay for services up-front or experience a delay in claims reimbursement.
Who are Global Core and GeoBlue?
Anthem’s Global Core program allows you to easily use your CSU benefits in Guam.
GeoBlue is the medical assistance navigator for Global Core. They provide core services such as the international provider network, claims support, and medical assistance.
When GeoBlue is contacted after you have made your appointment, but before the appointment date, GeoBlue will contact your healthcare provider to:
- Guarantee claims payment when the provider accepts payment directly
- Provide direct settlement with the provider eliminating the need to file claims yourself
- Obtain immediate authorization to avoid waiting for prior authorization
- Request you only pay your deductible, coinsurance or copay at the time of service
They can also:
- Help employees with international healthcare
- Translation services
- Member appointment and communication support
- Point of contact for international providers
- Monitors member inpatient care
- Arranges medically necessary transportation

What are the steps to accessing benefits?
On or around your date of hire
Step 1 – Review initial benefits information
CSU Human Resources ([email protected]) will send you an email containing details about benefits, when and how to enroll.
Within your first week
Step 2 – Meet with a benefits administrator
Meet virtually, one-on-one with an experienced benefits administrator. They will explain plan details, how to access care in Guam and provide information so you can decide what plan is best for you. They will be your dedicated, point of contact for benefits, now and in the future. They will contact you directly, but you can also contact our team at [email protected].
Within 30 days of your date of hire
Step 3 – Enroll in your benefits
Enroll through CSU’s Employee Self-Service, and upload official documentation to our secure OneDrive for eligible dependents you cover. Your effective date is the first of the month following your date of hire. Be sure you are connected through GlobalProtect first.
As of your effective date for coverage
Step 4 – Obtain coverage documents from HR
Anthem will provide a personalized provider guide for you to take to appointments. It includes:
- How to submit claims for guaranteed payment
- Directions on obtaining authorization, if needed
- GeoBlue contact information
You will also receive a member guide that includes:
- Your member ID, and plan details such as deductible and coinsurance
- Information on outpatient and in-patient services
Throughout the duration of your employment

Examples of member and provider guides
You will receive a personalized provider guide/letter (a confirmation of benefits) that will assist you to easily access care. Contact GeoBlue first and be sure to bring this letter to each appointment.
Employees who attend appointments without contacting GeoBlue first or do not bring their provider guide may be required to pay for services up-front or experience a delay in claims reimbursement.
How do I find care or providers?

On the Global Core website, you can find hospitals, physicians and specialists.
- Navigate to www.bcbsglobalcore.com
- Enter the first three letters on the Anthem ID card: OLU
- Select “Go”
- Select the “Provider Type” (e.g. Doctor, Hospital, Pharmacy)
- Search by Location, then select “Search”
You can also search for care and providers by using the Anthem Sydney Health app.
Anthem preferred providers
There are preferred providers in the GeoBlue network and Anthem guarantees streamlined claims processing at:
- American Medical Center (Mangilao/Upper Tumon)
- FHP Health Center
- Guam Medical Center
You can see any provider you would like, but preferred providers are familiar with the insurance process and ensure a streamlined approach.
When searching for a provider, they will be indicated on the website as a “network provider” if they are in Anthem’s network.

Other considerations
- When searching for a provider, a box with “network provider” will indicate if they are in the GeoBlue network.
- If they are not labeled as a “network provider”, they are not in-network. However, the non-contracted providers on the website have been vetted, meet quality standards and may accept a “guarantee of payment” on a case-by-case basis.
- When possible, you should make an appointment a few days in advance, then contact GeoBlue.
- GeoBlue will collect the information and send the guarantee pay letter/guide to the provider. With this method, you would only be responsible for applicable co-pay or deductible amounts at the time of your service.
- Anthem TeleHealth and Nurseline options are not available when residing in Guam. You must be state side for these services.
- If you are enrolled in the Ram Plan, you can use your HSA debit card from Fidelity to pay deductibles or co-payments.
- The VSP vision plan does not currently have in-network providers in Guam.
Dental
- The CSU dental plans have a “freedom of choice” option when it comes to providers. You can still receive benefits regardless of whether the dentist is in-network.
- If you need to obtain services, you will submit a claim afterward to Delta Dental of Colorado.
- You can either mail your international claims to the address below or email to [email protected].
- Include a receipt for services and a completed ADA claim form. If a currency exchange has been completed on the submitted fees, please indicate it on the form.
- Your dentist may require you to pay in-full at the time services are received.
