• W-2 reissue request form for current employees and former employees with an active eID/NetID
  (You will be asked to log in to verify your identity)
• W-2 reissue request form for former employees without an active eID/NetID
  (You will be asked to upload an image of your driver’s license to verify your identity unless you wish your W-2 to be mailed to the address currently in the payroll system.
