Consent to Receive Electronic Disclosure Notices
Celebrate your Ram pride and get on board with CSU’s Green Initiative by consenting to obtain information electronically; it is great to be Green! Accepting to receive documents electronically provides fast and secure access to your benefits and post-retirement information.
Your consent to accept information electronically remains effective until changed by you. If you do not consent to receive information electronically, you have a right to receive a paper version of the electronically transmitted document.
Electronic Acceptance of Disclosures and Notices
By submitting your electronic form, you consent to use electronic communications instead of paper. An individual who consents to receive retirement related documents from Human Resources electronically will not receive a paper copy. Understand that future communication and/or notices will be provided to you electronically.
Hardware, Software and Email Requirements
To access information or print and retain retirement information electronically you will need an email address, internet connection, web browser, and Adobe Acrobat reader. You must update your contact information in accordance with University procedures to keep your email address current.
Consent Acknowledgement
By submitting my form electronically, I hereby agree to electronic delivery of notices provided to me. I affirm that I have an email address along with the ability to access information in Adobe Acrobat Reader. I further understand that this consent is given to all statements furnished to me following the date of consent. My consent is also effective for the current calendar year and all future periods until I withdraw consent, in writing.
